For help navigating this wizard, follow these steps:

  1. Click the Get Help Now button anywhere on
  2. Read the overview screen and click Get Started.
  3. Type the name of your medicine into the search bar.
  4. Select your medication from the drop down menu and click Search.
  5. Then click Search.
  6. Click Add to My Medicines.
  7. Continue searching and adding the rest your medicines. When you're finished adding medicines, click Next.
  8. Tell us about yourself by completing the required fields (marked with *) and click Next. 
  9. Review your program matches and use the check boxes to select the programs to which you'd like to apply. Please note that clicking More Information will provide more details about each program match. Once you've s elected your programs, click Next.  
  10. Download your program applications by clicking Get Applications. If you prefer to have the applications emailed to you instead, simply enter your email address and click Email Me a Blank Application.
  11. If your applications did not download, click Download and we will try to deliver them again. Alternatively, you may enter your email address and click Email to have your applications emailed to you. If you need additional assistance, call us at 1-888-4PPA-NOW or 1-888-477-2669.
