The Louisiana Childrens Health Insurance Program, or LaCHIP, provides health care to uninsured children up to age 19. It is a no-cost health program that pays for childrens hospital care, doctor visits, prescription drugs, shots and more.

How to Apply:
Select one of the links below to download the application or go to the program site for more information on how to apply. Once you fill out your application, send it to the address on the application. Do NOT send it to PPA.
In order to qualify for LaCHIP, your child must meet the following criteria:
The child receiving health care must be under the age of 19.
The child may not currently be covered by any form of health insurance.
Total household income cannot amount to more than 200 percent of the Federal Poverty Level.
Household income does not include income of stepparents, grandparents or others living in the household.
The only way to know if your child can have LaCHIP is to apply.
