Arizona
KidsCare is affordable health insurance for children 18 years old or younger. KidsCare is provided by the State of Arizona and is administered by the Arizona Health Care Cost Containment System (AHCCCS).
All members of KidsCare (except Native Americans) must pay a monthly premium. KidsCare will cost no
more than $25 a month for one child or no more than $35 a month for multiple children.

Eligibility:
To qualify for KidsCare you must:
Be a resident of Arizona
Be 18 years old or younger
Be a U.S. citizen or a qualified eligible immigrant - regardless of the status of the parents
Not currently be covered by other health insurance
Not qualify for coverage through a state agency employee
Be a member of a household that meets the KidsCare income guidelines
Be a member of a household that is willing to pay a premium

Applying for KidsCare is easy!
You can apply by mail and no office visit or interview is required. If KidsCare needs more information for your application, they'll contact you by e-mail, regular mail, or phone. To apply, follow these simple steps:
1. Get an application.
2. Fill out and sign the application.
3. Pick a health plan.
4. Gather paperwork, like pay stubs, to send with your application.
5. Send your signed application and required paperwork to KidsCare.
